The ideal candidate will have the following:

• Bachelor's degree in social work or a bachelor's degree in human services field including but not limited to sociology, special education, rehabilitation counseling, psychology.
• One year of supervised social work experience in a health care setting working directly with individuals preferred.
• One-year current practical experience in utilization review utilizing InterQual Criteria, and quality improvement preferred.

Job Summary:

The case manager will incorporate the Hospital Values, Patient Centered Care philosophy, and goals set forth by the Board of Commissioners into all aspects of job performance.

Coordinates the organization of Utilization Review and care across the continuum. Identifies the need for medically related social services and identifies high risk and high resource requirements. The case manager works collaboratively with the healthcare team to achieve standards of care. The case manager serves as a liaison among all disciplines to coordinate patient transition from hospital to home.

Performance Responsibilities:

• Coordinates Utilization Review by utilizing InterQual criteria and intervenes as appropriate when changes in status are indicated.
• Performs comprehensive assessment through interviews with patients and primary support system to coordinate discharge planning.
• Match patient care needs to available resources, ensuring that patients stay at an appropriate level of care, making adjustments as necessary.
• Documents findings in a logical and comprehensive format.
• Facilitates communication and collaboration with all disciplines to optimize patient outcomes in all patient care areas.
• Facilitates processes to support patient's ability to access and utilize resources that will best meet their needs.
• Provides crisis services for the Emergency Department and for patients as needed.
• Arranges for obtaining home health care services and adaptive equipment as needed.
• Coordinates transfers to Assisted Living Centers, Skilled Rehabilitation Centers, Adult Family Homes, Specialty Hospitals and Hospice care.
• Identifies patients who are appropriate for Swing Beds, facilitates their admission, arranges for multidisciplinary care conferences, and works as a liaison between families and the care team.
• Provides medically related social work as needed in any patient care area.
• Demonstrates, Leads, and Mentors others in the delivery of our Hospital Values of Excellence, Partnership, Integrity, and Compassion
