Era Living is seeking Companion Case Management Assistants to become the newest members of our Resident and Family Supportive Programs team in Seattle, WA. Flexible schedules available - work on an on-call or scheduled basis up to 20-25 hours per week based on resident need.
Primary Responsibilities:
Under the direction of the Resident and Family Supportive Programs Director, and in collaboration with the community counselors, this role provides case management functions with assigned residents who may, for example, be experiencing memory loss, loneliness, anxiety, grief, and/or challenges with organization. Tasks could include:
- Helping to organize apartment and minimize clutter
- Reading
- Jigsaw puzzles, crosswords, Sudoku etc.
- Going for walks
- Accompanying to social activities

Knowledge, Skills, and Abilities:
- Ability to work effectively as part of a multidisciplinary team
- Competent interpersonal skills that facilitate communication and interaction with residents, family members, and/or other third parties
- Ability to foster a positive environment and represent Era Living in a proficient and professional manner; maintain professional boundaries and use discretion in consideration of stated visit objectives and Community Care Team objectives
- Ability to use computers, Word, Excel, Outlook and the Internet including smartphone applications
- Must possess a personal computer/tablet or smartphone with internet access and ability to access and utilize email and calendar (Outlook)
- Must be available to take 4 visits per month
- Ability to read, write and communicate in English, required
- Ability to pass DSHS background check, required
Education and Experience:
- High School or GED, required; Bachelor’s Degree in human services, social work, gerontology etc., preferred
- Experience working with seniors, preferred
- First Aid and CPR certified or able to obtain within 30 days of hire
- Complete Caregiver Dementia and Caregiver Mental Health training within 120 days of hire
