Position Summary:
This member of the PACT multidisciplinary team is responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals to community physicians, providing management and administration of medication in conjunction with the psychiatrist, and support services.
Responsibilities:
Provide service coordination (Case Management) for an assigned group of clients including coordinating and monitoring the activities of the Individual Treatment Team (ITT).
Assume responsibility for developing, writing, implementing, overall treatment goals and plans in collaboration with the client and registered nurse on assigned consumers.
Providing individual supportive therapy and symptom management, with the collaboration of the registered nurse ensuring immediate changes are made in the treatment plans as client needs change, educating and supporting clients, families, and advocating for clients' rights and preferences.
Participate in and conduct focused assessment of the following:
psychiatric history (i.e. onset, course and effect of illness, past treatment and responses, and risk behaviors)
Mental Health status and diagnosis
Physical and dental health
Use of drugs or alcohol
Education and employment
Social development and functioning
Activities of daily living (i.e. self-care, living situation, nutrition, money, management, family structure and relationships).
Consult with community agencies and families to maintain coordination in the treatment process
Provide on-call Crisis Intervention covering night time and weekend hours
Document client progress to maintain a permanent record of client activity according to establish methods and procedures.
Participate in daily staff organizational meetings and treatment planning review meetings
Participate in providing medication administration and medical services
Under the direction of the team psychiatrist and in collaboration with registered nurses on the team, develop, revise, maintain, and supervise team psychopharmacologic and medical treatment, medical policies and procedures including transcribing, administering, evaluating, and recording psychotropic medications prescribed by the team psychiatrist.
Evaluate and chart psychotropic medication effectiveness, complications, side effects and arrange for required lab work according to protocol.
Under the direction of the team psychiatrist and in collaboration with other registered nurses on the team, organize and manage the system of getting medication to clients, manage pharmaceuticals and medical supplies, integrating medication administration into clients' individual treatment plans.
In collaboration with the team psychiatrist, coordinate, schedule, and administer focused assessments of client physical health by making appropriate referrals to community physicians for further assessment and treatment to coordinate psychiatric treatment with medical treatment.
Participate in treatment, rehabilitation, and support services
Provide medication education and groups
On- call and medication monitoring
Perform other duties as assigned
Qualifications
Job Requirements:

Physical Requirements:
This position normally requires that physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision and skill.
Non-Physical Requirements:
Education
Obtaining Licensure with the Washington State Board of Nursing
Experience
Preferred 1 years' experience in an inpatient or outpatient psychiatric setting.
Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illness, and respect for client rights and personal preferences in treatment are essential.
Special Skills
Electronic Medical Records skills (EMR)
Good interpersonal skills
Possess and utilize effective verbal and written communication skills
Ability to provide service in a culturally competent manner
Ability to be self-motivated, work independently, and as a team member
Ability to represent the agency in a professional manner within the community
Bilingual (English/Spanish) preferred, but not required
Licensure, Registration, Certification
Must be licensed to practice as a licensed practical nurse within the State of Washington.
Valid Washington State Driver's license and minimum required liability insurance for WA State
Must be deemed insurable as determined by Catholic Charities' liability insurance provider
