About the Position
The Home Care Supervisor is responsible for overseeing and coordinating comprehensive care plans for assigned clients receiving in-home care services. This role involves assessing client needs, developing personalized care plans, coordinating home care services, and ensuring the delivery of high-quality care. The Home Care Supervisor serves as the primary point of contact for assigned caregivers, client caseload and their families, advocating for their needs and ensuring seamless communication between all stakeholders.
Essential Duties and Responsibilities
- Schedule and conduct new client assessments following agency policies and procedures.
- Conduct regular home visits and follow-up assessments to ensure clients' needs, preferences, and living conditions are kept to standard.
- Develop personalized care plans in collaboration with clients, families, and healthcare providers. Monitor the quality of care provided and address any issues or concerns promptly.
- Schedule and assign home care workers to clients; supervise and coordinate the daily activities of caregivers assigned to the caseload.
- Assist in developing, planning, and conducting orientation and training programs for caregivers.
- Conduct caregiver evaluations and ensure compliance with training and certification requirements.
- Address and resolve caregiver complaints and issues; administer disciplinary action for caregivers, as appropriate.
- Process and audit caregivers' electronic timekeeping for payroll purposes.
- Report, investigate, and document workplace accidents in compliance with agency policies and procedures.
- Make emergency arrangements for clients, including notifying family and collateral contacts.
- Respond to client emergencies and complaints, and coordinate appropriate interventions. Arrange for emergency services and notify relevant parties, including family members, healthcare providers and management.
- Manage and update client case files, provide daily documentation of any relevant information, and support internal and external audits of caseload.
- Collaborate with the marketing and sales team and the Director of Home Care Services to create and distribute marketing documents and materials throughout the community and to partners.
- Participate in community events and outreach. Advocate for, outreach to, and refer clients for home care services.
- Serve as a liaison to community agencies.
- Other duties as assigned.

Compensation:
$67800k-$75000k per year, depending on experience
Position Requirements

- Must have active driver license, clean driving record and vehicle.
- Proficient in MS Office Suite, e.g. Power Point, Excel, Word, etc.; and
- Bilingual is strongly preferred
Supervisory Responsibilities:
Provides direct supervision, training, and coaching of assigned caregivers. Escalated matters of significance are referred to Director of Home Care Services.
Education and/or Experience:
Highschool Diploma, GED or foreign equivalent required. Minimum of 1 year in health care requited. Prior experience in case management, care supervision or home care leadership is strongly preferred. Any other related experience and/or training, or equivalent combination of education and experience.
Certificates, Licenses, or Registrations:
Valid Washington State Driver’s license, satisfactory driver abstracts and proof of auto insurance. Must have the ability to successfully complete pre-employment and on-going background checks. CNA, HCA, LPN, RN, MSW licenses preferred, but not required.
Computer Skills:
To perform this job successfully, an individual must have advanced computer skills and demonstrated experience in Microsoft Office Suite, Word, Excel, and Outlook. Must have experience with or the technical aptitude to proficiently use health care programs.

Pay: $67,800.00 - $75,000.00 per year
